Aileen is a nurse practitioner who began her nursing career at Alexian Brothers Medical Center. While there, she worked as an RN and charge nurse as part of a primary rapid response team. In addition, she was the primary preceptor for new nursing students. Later, Aileen chose to build on her experience and began the advanced practice nurse program at the University of Illinois at Chicago. While there, Aileen also accepted a position as a float nurse in the NRO Department – ICU/stepdown track.
Alice Amedu, LPN
I began my career in nursing a little over eight years ago, but those close to me would say I had a thing for helping and caring for others as early as nine years old. I have worked in many different settings including longterm, short term and subacute care and as a cancer survivor myself, oncology is one of my biggest passions in nursing and where I can see myself in the future. Honestly I can say this is more than a job to me, and as the days go by I can say I'm happy with the decision I made to become a nurse. As MPAC's first LPN, I can't wait to see where this opportunity and career will take me.

Alice has received a post-masters certificate from Lewis University for Adult-Gerontology Primary Care Nurse Practitioner. Alice brings an extensive background in critical care medicine. She prides herself on providing evidence based and compassionate care. She has started her career as an ICU staff RN and Research Nurse Clinician. She then obtained a Master’s Degree from Rush University in Nursing and was a Clinical Nurse Specialist for an ICU. She then worked in the ICU of Hinsdale hospital where she was a staff RN and Clinical Coordinator.
Allan Boquiren, MSN, AGPCNP-BC
Allan is an Adult-Gerontology Nurse Practitioner who obtained is Master's Degree from Resurrection University. He started out as a patient-care technician and after obtaining his RN license, he worked in various settings such as skilled nursing and assisted living facilities, medical-surgical, telemetry, and oncology hospital units. He is certified by both the American Nurses Credentialing Center and by the American Association of Nurse Practitioners. He also holds a Bachelor's degree in Biology from the University of Illinois - Chicago.
Allen Patin, Jr.

As President of MPAC's Western operations, based in Southern California, Allen brings with him a wide range of operational and analytical skills. Prior to joining MPAC, Allen spent several years in investment management at Morgan Stanley, a global investment bank and brokerage. His most recent role focused on leveraging capital markets for private wealth investors following high value liquidity events. Additionally, he pioneered the brokerage's financial planning and analysis division, charged with monitoring investment performance analytics and structuring private and institutional portfolios based on market trends. Allen also has extensive experience in the non-profit sector, having launched and managed multiple organizations throughout the years
Alyssa graduated from Elmhurst College with a bachelor's degree in Accountancy. Her involvement on the track and cross country team drove her to pursue a second bachelors in Exercise Science. After graduation, Alyssa accepted a job at Wirtz Beverage as a staff accountant. Although her title remained the same, she progressed from working on their smallest sister company to their main Illinois location.
Amanda Lund, DO
Dr. Amanda M. Lund is a graduate of the Chicago College of Osteopathic Medicine (CCOM) at Midwestern University. She completed her residency and fellowship at University of Utah. Dr. Lund is board certified in Internal medicine, Geriatrics, and Hospice and Palliative Care. She is passionate about teaching and training the next generation of physicians and medical providers.
Dr. Lund is dedicated to helping patients and their families navigate the healthcare system, and utilize resources in a way serves what matters most to them.
Amber Painter, MSN, APRN
Amber began her nursing career as an Licensed Practical Nurse in 2005. While working her way toward an Associates Degree, which lead to a Bachelors Degree, nursing quickly became her passion and fueled her desire to grow and learn. Throughout this time Amber gained experience in long term care, critical care, oncology, emergency and trauma.
Amber earned her Masters of Science in Nursing in 2017 from the University of St. Francis in Joliet, IL. When she began this journey, Amber would never imagined it transpiring into the career she has today. She looks forward to the continued opportunity to provide care to past generations and being an advocate for patients, their families, as well as for the team caring for them every day.
Amie Burns- Sharnke, DNP, MS, FNP-BC

Amie has over 10 years of experience in the medical field starting as a Chemist in 2003. She became an RN in 2006, graduating from Purdue University- Calumet and started her career as an Emergency Room RN at Methodist Hospitals in Gary, Indiana. In 2008, Amie continued to expand her education and her career once again graduating from Purdue University- Calumet becoming a Family Nurse Practitioner. With a multitude of experience as a FNP, she enrolled at Valparaiso University and in 2013 earned yet another degree becoming a Doctor of Nurse Practice.
Angela Gard, APN, NP-C
Angela graduated from Purdue University Calumet as a RN and worked critical care, ICU, IMCU, CCU for 10 years including being permanent charge run of a busy 45 bed unit. In 2012, she transitioned as a NP, earning her MSN from Purdue University Calumet. As an NP, Angela has worked family practice, urgent care, rural health and sub acute care in rehab facilities.
Angela is married with two grown children and enjoys living in the peaceful country surrounded by her best friend and many pets.
Anita Shah, MSN, APN, FNP-C
Prior joining the MPAC team, Anita was a nurse practitioner at HCR ManorCare – Post-Acute and Rehab in Whitehall PA. In that role, she provided on-going medical management and acute interventions for patients in a post-acute setting. Before that, Anita honed her skills as a Nurse Practitioner in the emergency room at St. Francis Medical Center in Midlothian, VA. Anita holds a Bachelor of Science in Geriatrics and Gerontology from Ohio State University, a Bachelor of Science in nursing from Ashland University, and a Master of Nursing from the University of Toledo.
Ann Bouck, APRN-BC, MSN, FNP
Ann Bouck is nationally board certified, as a Family Nurse Practitioner with clinical experience from birth to death. Ann specializes in Geriatric Care, and is Palliative and Hospice Nurse Practitioner. She enjoys and is drawn to Orthopedic and Cardiac Care. Serving as a Clinical Instructor for University San Diego Doctor of Nurse Practitioner and California State University San Marcos Nurse Practitioner programs. Ann has previously served on the Nursing faculty of Biola University, California State University Stanislaus and California State University San Marcos. She brings a rich experience of Acute Care, Critical Care and Emergency Nursing into her clinical practice.

Ashley is a certified Family Nurse Practitioner. She obtained her Bachelors of Science in Nursing for Illinois State University and worked as a nurse on an inpatient medicine unit at Northwestern Memorial Hospital for seven years. During her time at NMH, she fulfilled several roles including charge nurse, preceptor, clinical coordinator, and Shared Leadership Committee board member. She went on to earn her Masters of Science in Nursing from Lewis University and is very excited to use her knowledge and skills as a member of the MPAC team.

Bonnie is an experienced nurse practitioner that started her nursing career over 10 years ago. She decided after working in the emergency department and the inpatient units to further her career and become a nurse practitioner through Olivet Nazarene University. She studied there for both undergraduate and graduate programs. She has experience as a nurse practitioner at Condell Medical Center as an acute care inpatient nurse practitioner, family practice nurse practitioner with an independent physician, and neurology and psychiatry through outpatient practice. Her passion is education, including patients, family members, nurses, and peers.

Brenda Smith is a Board Certified Family Nurse Practitioner. She received her formal education at Olivet Nazarene University in Bourbonnais, Illinois. She has an extensive background as a critical care RN while working at a level one trauma center in Olympia Fields, Illinois. She started her career as a nurse practitioner in a community-based clinic; specializing in health promotion & diagnosis and management of common and complex health problems.

Brittany Edward has a background in long-term care, medical-surgical nursing, critical care nursing, and most recently adult family practice. She received her Masters of Nursing from Indiana Wesleyan University and is a certified family nurse practitioner through the American Academy of Nurse Practitioners. Prior to employment with MPAC, she was practicing at an adult male prison - managing both chronic medical conditions and acute illnesses.
Cailian Mo, MSN, FNP-BC
Cailian Mo is a Nurse Practitioner who has a passion for the geriatric population, which has been strongly influenced by her culture, as the elderly are well respected in her homeland of China.
Cailian started her nursing career over 20 years ago. Her main clinical background is the Emergency Department. She also has rich experience in outpatient care, as well as inpatient care. She received her Masters of Science in Nursing at Olivet Nazarene University in 2016 and became a board certified Family Nurse Practitioner the same year. Cailian believes that excellent teamwork makes dream work.
Carrie Pappas, ARNP
Carrie has over 10 years of experience as a Nurse Practitioner with experience working in a variety of settings including Pediatrics, Hospice, and ER. She earned her undergraduate degree at Bowling Green State University and her Masters of Science in Nursing at the University of Toledo Medical Center. Carrie has experience working in a variety of settings including Pediatrics, Hospice, and ER. She currently lives in Sarasota, Florida and loves spending time with her family, traveling, and triathlons.

Cecilia Ann earned her Bachelor of Science in Nursing at Goldfarb College of Nursing. She has over 5 years of experience in the ICU, step-down unit and rehabilitation. She then obtained her MSN-FNP and joined the MPAC team where she gets to fulfill her dream of being a patient advocate and promoting wellness to the geriatric population

Dr. Cherie Gilleon is a board-certified family physician who attained her medical degree from Western University in Southern California and completed her Family Medicine residency at St. Joseph Hospital in Chicago.
Dr. Gilleon's honesty, humility, and respect for patients has won her a loyal and ever growing clientele. She provides her patients with the information they need in order to make good decisions for themselves, rather than making health decisions for them. Dr. Gilleon does her utmost to ensure that all patients fully understand their own health issues and recommends treatment options or preventative care based on the patient's own preferences, plans and goals.

Christian started his career in healthcare as a certified nursing assistant at Glenbrook hospital. While there he, developed a thirst to become a healthcare provider and help those in need. He went on to become a registered nurse in the Coronary Care Unit at Northwestern Memorial Hospital. Christian earned both of his degrees, BSN and MSN, at North Park University. He is adamant about providing the same excellent healthcare, suitable for his own family, to all individuals.
Christie Marie Angderson, MSN, APN, FNP-C
Christie Marie began her nursing career as a Nurse Care Technician on a Medical Telemetry/Renal Unit at Advocate Lutheran General Hospital. From this, she was exposed to the science and art that is Nursing and was inspired to pursue her Bachelor's of Science in Nursing at Chamberlain College of Nursing. After graduation, she started as a Registered Nurse on the same unit, learned, and worked her way to become one of the units' Charge Nurses. She was also a resource and preceptor for New Graduate Nurses/New Orientees. Christie evolved her nursing craft around being the patients' advocate and providing genuine holistic care. Because of this, it pushed her to pursue her Master's in Science of Nursing degree also at Chamberlain College of Nursing with a focus on being a Family Nurse Practitioner.
Prior to joining the MPAC family, Christie worked as a Family Nurse Practitioner at a Pediatric/Adult Clinic in the suburbs. She also works as a Clinical Instructor at Chamberlain College of Nursing. Courtney is a Licensed Clinical Social Worker. She earned her bachelors degree in Psychology from Michigan State University and her Masters of Social Work at Loyola University Chicago.
Her past experience includes providing therapy in residential and private practice settings, Assessment & Referral and Utilizations Review in the hospital setting, medical social work in the Caribbean and case management in long term care.
Courtney is passionate about making differences in the lives of others. She believes that quality healthcare should be the expectation and is actively working toward that with new program development for LCSWs at MPAC.

Diane is a caring and compassionate provider. She enjoys taking care of her patients and their families with a holistic approach. She joins MPAC with 10 + years of nursing experience. She graduated from Bradley University with a BSN and started her nursing career as a ICU nurse at Alexian Brothers Medical Center - during which she was staff nurse, charge nurse, mentor and member of the rapid response team.
As she looked to grow and expand her nursing career she completed her MSN-FNP in Dec 2009 from Northern IL University . Working as a APN for the last several years she has had the opportunity to provide care for a variety of patients in the areas of wound care, post acute care and primary care - delivering quality patient care as well as being an advocate for illness prevention through patient and family education.
Diane enjoys working with the geriatric population and looks forward to learning new things and working as a team member with her colleagues.
Elena Petrusevska, PA-C
Elena came to MPAC with a broad range of experience in the areas of chronic disease and pain management. In addition to working for an internal medicine clinic, Elena also spent two years rounding in nursing facilities. Elena graduated from Lewis University with a Bachelor of Science in Biology earning Cum Laude honors. She then went on to earn a Masters in Physician Assistant Studies from Midwestern University College of Health Sciences.
Fran is an adult gerontology nurse practitioner who began her nursing career at Bromenn Regional Medical Center. While there she worked as an RN in the ICU step down. She then went on to Northshore University Health Systems followed by Northwestern Memorial Hospital in the telemetry units. She acted as a charge nurse when needed and played an active role in training and mentoring new hires. Fran joined the MPAC team upon completing her advanced practice nurse education at the University of Illinois Chicago.

Dr. Gwen Pessis Gola is the National Director of Business Development at MPAC. Dr. Pessis Gola brings a diversified wealth of healthcare knowledge to MPAC, having spent her career in academic, research, clinical and business roles. Gwen is a published researcher, has taught neurological clinical skills classes at The Chicago Medical School/FUHS and has managed rehabilitation departments at various facilities across the country. Dr. Pessis Gola completed her undergraduate degree at Indiana University, her Doctorate of Physical Therapy at The Chicago Medical School/FUHS and was trained in neurological rehabilitation at the Rehabilitation Institute of Chicago. When not working, Gwen enjoys spending time with her family and friends and competing in triathlons.

Heather graduated from University of Illinois at Chicago with her APRN degree in December 2015. She began her nursing career in 2002 after graduating with her BSN from Rush University. Heather was a bedside nurse for 15 years caring for the adult and geriatric population diagnosed with hematological cancers. Heather worked in tertiary centers in Denver, CO and at Rush University Medical Center in Chicago, IL. Heather is always looking to add to her nursing and medical knowledge and enjoys teaching peers and patients.
Iesha Coleman- Nwagwu, MSN, NP-C
Iesha earned her Bachelor of Science in Nursing at Chicago State University and has over 5 years experience in long- term care, medical-surgical and outpatient settings where she functioned in the role of charge nurse. Iesha has gone on to receive her Masters of Nursing from Chamberlain University and is certified by the American Academy of Nurse Practitioners as a Family Nurse Practitioner.

Irene-Nicole Lucero was born and raised in San Diego. She received her Bachelor of Science in Nursing at West Coast University in Orange County. She started working at Jacob Healthcare center before working full-time at Anaheim Regional Medical Center as a critical care nurse. After working two years at Anaheim Regional Medical Center, she got accepted to Azusa Pacific University for their Master of Science in Nursing and Family Nurse Practitioner program in San Diego. While attending graduate school full-time, Irene-Nicole worked as a PCU nurse at Sharp Grossmont and Sharp Memorial Hospital. Irene-Nicole loves helping people and hopes to join more medical missions in the near future.

Jason took on his first entrepreneurial venture at the age of 23 by assuming the lease of a struggling coffee shop. The venture proved to be only moderately successful, however, the lessons Jason learned as a young business owner became guiding principles that are reflected in every important business decision he has made since.
Jason went on to enjoy a successful decade real estate development and sales. Along the way he became a Regional Sales Manager for Heller Development responsible for the sales and marketing of 3 master planned communities.
In 2011, Jason was inspired by an opportunity to innovate in the healthcare industry. He joined forces with his wife, who is a physician and together they started a multifaceted medical practice. Building on initiatives set in motion by healthcare reform, Jason focused his attention on developing a program capable of delivering medical care across the post-acute spectrum. That program laid the foundation for the company that is now MPAC, of which Jason is a founding member and the Chief Executive Officer.
Janet Zedler, FNP-C

Janet earned a Bachelor of Arts Degree from UCLA with a major in Psychology in 2007. She then entered Western University of Health Science’s, master’s entry into nursing program where she earned her Master of Science Degree in Nursing and her post master’s nurse practitioner certificate in 2012. She is board certified as a Family Nurse Practitioner by the American Academy of Nurse Practitioners.
Janet worked as a RN on the oncology unit at Huntington Memorial Hospital until she obtained her FNP credential and has spent the last four years working as an FNP with MinuteClinic®, the country’s largest retail health center in the US.

Josalynn Brown, MSN, APRN, FNP-C
Josalynn is a highly ambitious, nationally-certified nurse practitioner with over 11 years of both clinical and managerial experience in the nursing field. In 2014, Josalynn earned her Master's of Science in Nursing at Olivet Nazarene University, specializing as a Family Nurse Practitioner. Leading up to her experience as a Family Nurse Practitioner, Josalynn acquired experience on several units including Progressive Care, Cardiac Telemetry, and ER. Prior to her time at MPAC she was also an NP at Heartland Manor Care Health Services-East.
Katie graduated from the University of Nebraska- Lincoln with a Bachelors of Journalism, focusing on advertising and public relations. Driven by a passion for music and performing, after graduation she focused on freelance branding and marketing with local musicians. MPAC is her first step towards her professional career.
Karen Panaganiban, MSN, AGPCNP-BC
Karen has over 14 years worth of experience in various inpatient settings. She began as an ER Staff Nurse at Presence St. Mary and Elizabeth Medical Center where she soon became a Telemetry Nurse. In 2016, she earned her Masters of Science in Nursing (MSN) at Resurrection University-Chicago, becoming an Adult-Gerontology Nurse Practitioner. Karen has experience working in many departments within the healthcare field.
Kathy Cacho, MSN, AGPCNP- BC
Kathy is a Board Certified Nurse Practitioner who brings 8 years of nursing experience in subacute care to her advanced practice. Her devotion to nursing and love of learning helped her earn a Master’s Degree in Nursing from Resurrection University. Kathy has a commitment to the continuity of the patient experience. She places a strong emphasis on developing relationships with patients as they progress, ensuring that her patients know they are truly cared for and that their outcomes genuinely matter.
Kimberly Grzesik, MS, APN, AGACNP-BC
Kim began her nursing career at Rush University Medical Center, where her patient care interests included cardiovascular and thoracic surgery, cardiac step-down, heart failure/cardiac transplant ICU, and neuroscience ICU. While at Rush, she earned a Master's of Science in Nursing and was active in staff education. Her next position was at Advocate Christ Medical Center, where she focused on the care of acutely ill as a member of the Rapid Response, Code Blue, Chest Pain Alert, and ED STEMI teams, as well as serving as an expert clinical resource for the identification and management of cardiac arrhythmias in the Central Telemetry Center.
Kim earned the Adult-Gerontology Acute Care Nurse Practitioner Post-Master's Certificate from the University of Illinois at Chicago. She is certified by the American Nurses Credentialing Center.
Kristin King, AGNP-BC
Prior to joining MPAC, Kristin worked as a Nurse Practitioner providing care in the post acute/rehab setting. She managed the primary care of patients and also specialized in pulmonary medicine while caring for chronically ventilated patients.
As an RN, her experience was focused on the intensive care setting at Saint Joseph Hospital in Chicago, IL. For four years, she had the invaluable opportunity to work as a travel ICU nurse. Kristin also worked in several diverse and challenging environments throughout the Chicagoland area. Kristen is a family nurse practitioner who completed her graduate studies at the University of Cincinnati. Prior to working with MPAC, Kristen worked primarily with heart and lung transplant patients at Northwestern Memorial Hospital, as well as UCSF Medical Center in the cardiac step down setting. She loves working the post acute care setting because she is able to help care for and educate the geriatric population.

Prior to Joining the MPAC team, Ladi worked at Mount Sinai hospital in a critical care unit for eight years. He received his Masters Degree in Nursing from Governors State University and is currently enrolled in coursework to earn a Psychiatric Mental Health Nurse Practitioner Certificate at Northern Kentucky University. Ladi brings compassionate care wrapped in enthusiasm and an insatiable appetite for learning, sharing and caring.
Lauren Fernberg, MSN, FNP-BC
Lauren Fernberg is a family nurse practitioner from New England. She obtained her RN and Master of Science in Nursing at Simmons College in Boston, MA, and also holds a Bachelor of Science in Health Sciences from Stonehill College. Lauren began her nursing career as an RN in a skilled nursing facility, where her primary experience was working with subacute medicine patients. Prior to joining MPAC, Lauren also worked as an RN at Emerson Hospital on a post-acute care rehabilitation unit where she was able to help patients recover and transition safely back to home.
Liza Alvarado, MSN, RN, PHN, FNP-BC
Liza Alvarado started her journey in nursing as a nurse's aide at a psychiatric skilled nursing facility, and at Rady Children's Hospital of San Diego. While working as a nurse's aide, she decided to further her education by earning her BSN from San Diego State University. She then worked as a registered nurse for several years in Surgical Oncology for Scripps Mercy Hospital, and then Acute Care, and Critical Care for Sharp Healthcare. She enjoyed mentoring and precepting other nurses, and providing evidence-based, compassionate, holistic care to her patients. She desired to make more of difference with her patients, and went on to earn her MSN and FNP from CSU Dominguez Hills. As a Nurse Practitioner, she has worked in a Private Family Practice, Urgent Care, and as a Hospitalist. She has joined MPAC to continue to provide that same compassionate, evidence-based care to her Post-Acute Rehab patients.
Marie Zelhart, MSN, APN, AGNP-BC

Marie began her RN career at Adventist Hinsdale Hospital as a medical/surgical nurse. During her time there, she was a precept for new nurse and a charge nurse on the weekends. She then decided to attend Purdue University to further her career and become a Nurse Practitioner. While in graduate school, she became a Clinical Instructor for a Nursing Fundamentals Clinical course at Purdue for sophomore nursing students. After graduating, she worked with the hospitalist team at Hinsdale and La Grange.
Maria Luisa Jabido, APN
Luisa has over 20 years of nursing experience. She started her career as a staff nurse at different long term care facilities. After several years, she transitioned into inpatient care as a staff nurse in the ICU at Glenbrook Hospital for almost 17 years. She received her Master's of Nursing from South University in 2016 and is a certified adult/gerontology primary care nurse practitioner. Luisa believes that quality care should be evidence-based and responsive to the patient's needs. She strives to take a personal approach in collaborating with patients and family members to provide optimal care.
Mary Ann Cantore, APN, FNP-C
Mary Ann is a Family Nurse Practitioner who earned her Master of Science from North Park University in Chicago, IL. She holds a national certification with the American Academy of Nurse Practitioners. During her training as a Nurse Practitioner, she developed a special interest and passion for geriatric care.
Prior to joining MPAC, she worked as a Pediatric Registered Nurse for 11 years in critical care, the majority of her career with Ann & Robert H. Lurie Children’s Hospital of Chicago (formerly Children’s Memorial Hospital).
Mary Ann Jones, MSN, FNP-C

Mary Ann received her Bachelor & Master's Degree in Nursing from Governor State University. Later, she attended University of St. Frances and received her Post Master degree as a Family Nurse Practitioner and certificate as a Legal Nurse Consultant. Currently, Mary Ann is pursuing her Doctorate from Chamberlain University. She is a member of NBNA, CCNBNA, CHI-ETA-PHI, SIGMA THETA TAU and PHI-THETA KAPPA,
Her love for nursing began in the long-term care setting. She was a clinical nurse specialist for over 10 years. She also fulfilled the roles of ADON, DON, Infection Control Coordinator, Unit Manager and Supervisor. Mary Ann is well-versed regarding IDPH State & Federal Regulatory Guidelines and dual certified as a wound care & diabetic wound care specialist by WCEI. Mary Ann believes in quality healthcare services and has made it her goal to continually educate nurses in the LTC setting on measures to promote safe healthcare services.
Melissa Pessis, MSN, FNP-BC
Originally from Minneapolis, MN, Melissa completed her undergraduate degree from University of Wisconsin-Madison. After 3 years in the Healthcare Marketing Industry, she made a career change into nursing. She attended Rush University's General Entry Master's Program and began working at Northwestern Memorial Hospital. Over the last 7 years, she has worked in both medical-surgical and emergency room nursing. Melissa has worn many hats as a Registered Nurse, including Preceptor, Charge Nurse, Clinical Coordinator and Education Coordinator. Most recently she graduated from Maryville University with a graduate certificate as a family nurse practitioner.
Melissa now lives in Highland Park, IL with her husband, twin daughters and dog Penny.
Chief Financial Officer
Mike serves as MPAC LLC’s Chief Financial Officer to oversee the development and execution of a financial strategy that coincides with the Company’s operational and strategic goals.
Prior to joining MPAC LLC, Mike was a Director at Conway MacKenzie, a national premier financial advisory firm, where he specialized in providing turnaround and crisis management, and financial restructuring services to distressed businesses. Mike served in various capacities while servicing clients including cash management, operational analysis, and financial modeling. Mike also spent five years at KPMG LLP, a Big Four global accounting firm, where he performed and coordinated financial statement audits for both public and non-public financial services and insurance clients.
Mike holds both a bachelors and master’s degree in Accounting from Western Michigan University. Mike is a Certified Public Accountant and has passed all parts of the Certified Insolvency and Restructuring Advisor exam.
Nicole Schott, AGNP-C

Nicole Schott received her Bachelors Degree in the Science of Nursing at Goldfarb College of Nursing in St. Louis, Missouri. She worked in a Surgical Neuro ICU at SSM for 3 years. In that position, she became a preceptor for new RN's joining the unit. After this position, Nicole worked as a critical care float RN while completing her Masters. She received her Masters in the Science of Nursing at the University of Missouri - St. Louis. She specialized in Adult-Geriatric Nurse Practice. Nicole recently moved to San Diego and worked as a traveling RN at USCD Sulpizio. Her goal as a Nurse Practitioner is to always provide safe and effective care with compassion.
Oluseun Osinowo, MSN, FNP-BC
With 8 years of direct critical patient care, Oluseun believes, practices and provides care using a holistic approach. He started his career in 2005 at Loyola Medical Center in Maywood, IL as a Staff RN in the Intensive Care Unit. In 2015, Oluseun completed his Master's of Science in Nursing specializing in Family at Governor's State University.
Pamela Sobrano, MSN, FNP-BC
Pamela has worked over seventeen years as a nurse, mostly in ICU settings, before becoming a Nurse Practitioner. She received her Bachelors of Science in Nursing from the University of Illinois-Chicago and her Masters of Science in Nursing from Northern Illinois University.
Prior to joining MPAC, Pamela worked in a community clinic as a Family Nurse Practitioner and later in Physical Medicine and Rehabilitation. She is driven by a desire to provide the best quality care she can offer, and her experience and passion has given her a strong interest in working with the geriatric population.
Rashida Lee, AGPCNP-BC
Rashida Lee has been a Registered Nurse for 10 years. She has experience in the areas of the medical-surgical, pre-operative & post-operative care and long term acute care.
Before joining MPAC, Rashida worked for Caremore Touch for over three years. In that position, she provided chronic and acute care to patients age 65 and older and gained tremendous experience in managing the complex care of skilled patients.
Rashida is ardent about the education of her patients, families, facilities, and herself.
Rachel Janesky, FNP-C
Rachel graduated with her Registered Nurse degree from the University of Southern Indiana in 2005. She then obtained her BSN and MSN through Indiana Wesleyan University. She is certified by the American Academy of Nurse Practitioners (AANP). She began her nursing career in a Telemetry/Pediatric unit and then transferred to ICU for 5 years prior to obtaining her Nurse Practitioner and now works in Adult Medicine. Since working as a Nurse Practitioner in 2014 she has cared for ages 15-geriatric in addition to rounding and caring for the geriatric population at 7 surrounding Long Term Care Facilities.
Rachel and her spouse reside in a small town in Eastern Illinois near Champaign along with their two young daughters.
Ruth Fernandez, MSN, APN, FNP-C
Ruth was a Registered Nurse for over 21 years before obtaining her Master's Degree from Walden University. A majority of her RN years were spent working in various Subacute Nursing Facilities and Long-Tern Care settings. Over her career, she has accumulated extensive knowledge on wound care management, excellent leadership skills and building patient/family relationships. Ruth's passion is caring for the elderly population and this is why she gravitated toward the position with MPAC.
Samantha Hyche, MSN, FNP-C
Samantha Hyche is a graduate of Governors State University. Prior to coming to MPAC, Samantha worked as a Transition Care Coach (TCC) at West Suburban Hospital. Here she helped decrease readmissions of Heart Failure, MI, Pneumonia , and patients with COPD. Prior to her role as a TCC, she worked at Vanguard Home Health. At Vanguard she managed patients with chronic illnesses and focused on preventing hospital readmissions. Samantha also worked at the University of Chicago Hospital as an oncology nurse. She loves working with people and aims at encouraging patients to be proactive regarding their health through education, support and health management. She believes in holistic patient care in order to get the best possible outcome for her patients.
Sara Will, MSN, APN, FNP-BC
Sara Will is an experienced nurse practitioner with speciality in internal medicine, physical medicine and rehabilitation, skilled rehabilitation, and retail health.
She has learned the art of caring for patients across the life span, however she has a deep connection with geriatric medicine. Sara brings passion for education to the table, not only to patients but to family members, nurses, and peers. She is a true patient advocate and strives to provide excellent patient care.
Shauna Quevedo, FNP-BC
Shauna is a Family Nurse Practitioner who began her career in the Intensive Care Unit at Alexian Brothers Medical Center in Elk Grove Village. While operating in multiple roles as a Charge, Mentor and Rapid Response Nurse, Shauna pursued her Master's Degree specializing in Family Practice at the University of Illinois-Chicago. Prior to joining MPAC, Shauna worked at a family practice in the Chicagoland suburbs. Her primary focus and goal is to provide holistic and compassionate care to all.
Sharone Perlman, RN, MSN, FNP-BC
Sharone completed her undergraduate education at the University of Minnesota and Family Nurse Practitioner training at Walden University. Her experience, education, and training has prepared her to care for patients of all ages. She worked for 15 years in the Acute Care setting, 10 of which were in the Emergency Department. This experience has given her the ability to quickly identify potentially life-threatening conditions and improve long-term outcomes for patients. When it comes to her profession, she attempts to build partnerships with her patients to help them meet their personal health care goals and educates her patients along the way so they better understand their condition and its treatment.
Stephanie Vasquez, MSN, FNP-C
Stephanie Vasquez is a board certified Family Nurse Practitioner from Southern California. She received both her BSN and MSN at California Baptist University in Riverside, CA. As a long-time lover of learning, Stephanie most enjoys educating patients learn more about their health and how they can promote their own well-being. Prior to becoming a Nurse Practitioner, Stephanie worked as a RN in the ICCU at Community Hospital San Bernardino, and also as a nurse manager of group homes for developmentally disabled adults for Rescare Inc. Stephanie seeks to promote the health and well-being of each of her patients holistically, so that they might be able to live their lives to the fullest.
Sonja Enge-Gilton, MSN, APN, FNP-BC
Sonja has over 12 years of experience in the healthcare field ranging from long term care, to Emergency medicine. She is self-motivated and dedicated to preventive care through patient/family education. Her greatest passion is caring for others and helping them to improve their health outcomes. Sonja believes in living life to the fullest and helping others to do the same. In addition to caring for others, she loves to spend time with family/friends, teaching, reading, running, and cooking.
Susan Pastor, AGNCNP-BC
Susan graduated with distinct honors from North Park University, specializing in Adult-Gerontology. Prior to joining MPAC, she worked at NorthShore University Healthcare in both inpatient and outpatient settings. She also worked at Northwestern Memorial Hospital with transplant patients. She has enjoyed establishing leadership skills working as a nurse and charge nurse for almost a decade. She strives to provide patients the best care possible
Telva Urban, APN-BC
Telva Urban has been an RN for 40 years and a NP for 14 of those years. She has a variety or nursing experiences including med/surg, chronic dialysis, acute rehab and working with homeless adolescents. She excels at building therapeutic relationships with chronic clients over time. She feels that all of her previous work has prepared her for doing work with SNF patients.
Tifany Jamison, APN, FNP-BC, MSN
Tifany Jamison, Ed.D, APN, FNP-BC, MSN is a board certified Family Nurse Practitioner. Prior to joining MPAC she served as Interim Manager and a Care Coordinator for the Population Health Department at Loyola University Medical Center. Tifany has worked in both clinical and academic settings, and enjoys working with people across the age spectrum. Her goal is to provide safe, holistic quality care while serving as a patient/family advocate.
Tiffany Thomas, MSN, APN, FNP-BC
Tiffany Thomas started her healthcare as a certified nursing assistant at a long term care facility. She then went on to Prairie State College where she received her Associate Degree in Nursing and began working as a Registered Nurse at Methodist hospital. She later received her Bachelor & Master's Degree in Nursing from Governor State University.
Tiffany has an extensive background as an Emergency Room Registered Nurse at St. James Olympia Field and University of Chicago Hospital. She has spent the last 3 years working as a Family Nurse Practitioner in a primary and urgent care setting. She is passionate about providing safe quality health care as well as educating her patients and staff.
Timothy J. Martinez

Prior to launching MPAC, Tim spent over 10 years in management consulting working domestically and internationally with companies in various stages of their life cycle. Most recently, Tim was a Director in the Chicago office of Conway MacKenzie, a nationwide premier financial advisory firm. Tim's role involved the financial and operational management of turnaround and restructuring strategies for companies in various industries and stages of distress. In addition, Tim has held interim financial and operational leadership roles with multiple clients over his career. Tim left Conway MacKenzie to become a founding member of MPAC and serves as its President and Chief Operating Officer.
Tim holds a Bachelors degree in International Business and Accounting from St. Xavier University and an MBA from the University of Louisiana, Lafayette. Tim is also a Certified Public Accountant and a Certified Insolvency and Restructuring Advisor.
Tracy Nix, MSN, RN, NP-C
Tracy has been a Registered Nurse for the past 7 years. She has experience as a floor and charge nurse on Cardiac Telemetry, has worked as a Stroke Coordinator, and most recently was a nurse in the Emergency Department. With a strong passion for nursing, Tracy furthered her education by obtaining her Master's of Science in Nursing and is a board certified Family Nurse Practitioner through the American Academy of Nurse Practitioners. Her main goal in healthcare is to offer compassion for those that are sick while promoting overall health and wellness.
Whitney Owens, MSN, APN, FNP-BC
Whitney is a family nurse practitioner who began her nursing career at Northwestern Memorial Hospital on a medical telemetry unit. While working at Northwestern, she acted as charge nurse and a mentor to new hires for over two years. She then continued her journey as a travel nurse gaining experience in different areas of nursing including hematology/oncology and neurology. Whitney joined the MPAC team after graduation to pursue her passion in geriatrics and provide high-quality, individualized care.
Tristian Talua, MSN, APN, FNP-C
Tristan began his nursing career after graduating with a BSN in 1997 and he takes pride with the years of nursing experience since then. His work includes Med/Surg, Cardiovascular-Intensive Care, Geriatric, Home Health and most recently step-down critical care with Alexian Brothers Medical Center in Elk Grove Village. He obtained his MSN-FNP degree from Olivet Nazarene University and was privileged to be invited to join the Sigma Theta Tau International Honor Society.
Young Shin, PhD, APN-C
Prior to joining the MPAC team, Young was a nurse practitioner at Hollywood Presbyterian Medical Center. She was also an assistant professor for graduate nursing programs at Azusa Pacific University for several years. Young holds a MSN and a PhD degrees in nursing from University California Los Angeles. Young has been a RN since 1991.
Zivile Calderon, MSN, APN, FNP-C
It wasn't a surprise that Zilvile quickly settled into her role at MPAC considering she spent the first seven years of her nursing career working as an RN in the fast paced, high pressure emergency room at Resurrection Medical Center. In addition to the Bachelor of Science in Nursing she earned at Benedictine University and the Master of Science in Nursing she earned at Resurrection University, Zivile also holds a Law Degree from Vilnius University in Vilnius Lithuania.
